What To Know About The Current Parasite Outbreak

An ongoing outbreak of cyclosporiasis—an intestinal illness caused by the microscopic Cyclospora cayetanensis parasite—is spreading across the US. The illness triggers watery and “explosive” diarrhea, stomach cramps, nausea, and fatigue. It is not spread person-to-person but by consuming contaminated raw produce or water.

Symptoms and Duration

The incubation period is typically about one week, though symptoms can begin anywhere from a few days to two weeks after exposure. Symptoms include:

  • Debilitating watery diarrhea
  • Severe bloating and gas
  • Abdominal cramping and nausea
  • Fatigue and a loss of appetite

If left untreated, these symptoms can last anywhere from a few days to a month or longer, and can lead to severe dehydration requiring hospitalization.

How It Spreads

You can become infected by eating or drinking items tainted with the parasite. Past outbreaks and current investigations have heavily linked the parasite to contaminated raw fruits and vegetables, such as packaged lettuce, cilantro, basil, snow peas, and berries. It does not spread through casual person-to-person contact.

Diagnosis and Treatment

To confirm a diagnosis, doctors must specifically request a stool test that uses a special stain to see the parasite’s eggs. Unlike many common stomach bugs, cyclosporiasis is treated with prescription antibiotics—typically a combination of trimethoprim-sulfamethoxazole (e.g., Bactrim or Septra) for 7 to 10 days. Hydration is also a critical part of recovery.

Prevention

Because Cyclospora is resilient, basic rinsing isn’t always enough to remove it, and the definitive way to eliminate the parasite is by cooking food to 158°F (70°C). To reduce your risk while handling raw produce:

  • Wash your hands with soap and water for 20 seconds before food prep.
  • Scrub fresh fruits and vegetables under running water.
  • Refrigerate produce promptly after buyin
